all of it, although ive only heard of one person ever losing these settings. and he just reset the device and they were back.

I would just use a camera and take pictures of all the screens so you can refer to them in case some is awry. No need to write them down as such. The chances of something happening are pretty low, but this is a good precaution to take.

yes, you would flash it the same way you would an OS ROM,
but why would u need to flash a new radio each time you update your OS ROM?
