After this, all audio is lost on the device, even after the call is dropped: No bluetooth, no earpiece, no speaker, no 3.5mm, no extUSB, etc. A reboot is required to reset audio, and I've only tried cold boots. Audio is reproducibly broken after repeating the sequence.
Further testing showed that this problem was caused by badboy8813's AudioPara3.csv. Returning to the original AudioPara3.csv resolved the problem. Bluetooth is working normally now.

Never figured it out, but I honestly didn't put much time into it. Suggestions welcome.

Let me know if/how I can help troubleshoot. Perhaps studying log file after attempting to toggle netloc on/off? Do I need to increase verbosity level of logging? If so, how do I do that? Any idea of what I would be looking for in the log file?

Verbosity would have to be increased by hacking at the source, I'm not aware of any other way to increase verbosity.

I thought someone replaced the APK and it worked - I swear I tried this, and no effect. It's been a while since I looked at it tho.

Can you post or do you have a link to an APK that I can try? I've been unable to find one by searching.
