OK...I think I know your problem now...in the first image you have a box unchecked (un-labeled) that is the 'bars'. You need to check it, then you should be good-to-go!!! Let me know how it goes!!!

Try and move the phone option...I am not sure what it is, but I would move it up above the unlabeled spot first, and then uncheck it if that doesn't dop the trick.

OK....I am running out of ideas here...try to install this CAB and see what your menu looks like (you can always uninstall it). I am not sure if it will fix it, but I figure at this point it can't hurt.

Before you do...are you SURE you don't have any profiler app installed that may be interfering with it?

Hmm this happened to me before. In your first picture where the icon gets all mis-aligned. I had that problem when i tried to mock with the menu and removed items on the tree.

Reinstall SPB fixed it.
