I use Spb Pocket 4 for the tabs on top and went into their official forums to find the icons.

For the black taskbar it took me a while to find it. I unfortunately forgot where I got it but I will attach it in my post.

Can you upload your dialer skin please? And are you using Pocket Plus X-Button or the default HTC one? For some reason my X button (using either one) is a light grey color, but I wish to change it to black like yours. Any ideas?

For the close button I used UITweaker to modify the color
