It sounds like either the download was corrupted somehow or he is using the sprint ROM for a VZW phone or vise versa.

No no, not carrier...

It sounds like a bad flash, bad XIP, bad Sys files. I would look at reflashing with a fresh kitchen download at all defaults except carrier.
