I have flashed three times. The And each time I had the same issue. When the UpdateUtility says it failed, disconnected the phone from the USB cable and reconnect it. Then run the utility again. This has to do with, from my experience with the instructions that are given on these pages. The ROMUpdateUtility will put your phone into the flash mode. Go to RUU folder (I put it on the desktop), run the RomUpdateUtility (you must already have your unzipped your Rom of choice in this same RUU folder, it is named RUU_signed.nbh). The ROMUpdateUtility will automatically will go through the proper steps, including telling you when to connect your USB cable. When the screen gives you instructions on things to do before going to the next step, then you want to make sure the phone is connected. It's worked each time once I have just followed the instructions on the ROMUpdateUtility.
