can you run IOTA from the phone settings?
start, settings, personal, phone, services tab:
then select Internet, get settings, and click Start and see if IOTA runs.
if not, get the IOTA cab and install it, run IOTA again and see what results that gives you.

might help to know what carrier you are using.

just tired that and getting the same results,
Data Provisioning Failure
error code 1012
Could not prepare Visioin services.

OK, lots of times thats just a network error due to the network being busy, or you can try logging in online and changing your Vision password- thats helped me in the past. it sort of forces the provisioning to refresh itself then.
