ADB is the Android Debug Bridge and is found in the Android SDK. Little more explanation here: http://android-dls.com/wiki/index.php?title=ADB Commands are explained here as well such as adb shell.
This is run from your computer, PC or Mac, in terminal or the command prompt. It is used to do things on your device from your computer.

No, you don't need to be in recovery. Just connect to your Mac and you should be able to use adb. As long as your phone is recognized anyway.

Computer should recognize it. As, in you can see it in device manager (I know, Windows thing but it should be similar in Mac).
