Question Swapping out phone with custom rom. Just restore first?

About 3 weeks ago this dime sized circle appeared on my screen. It looked like there was a sepia overlay in a circle distribution in only that spot. It was there no matter what I did, the colors were just almost inverted under it. The touch worked etc.

Long story short that disappeared but you can see a rainbow sorta square when the screen isnt on now. I talked to the guy at verizon and he said call the help number and as for an exchange. They'll send me one, anyway do I just need to hit restore to custom defaults on the phone first?

I made a point to not 'unlock' the phone when I flashed the custom rom on there. Is this all I need to do?

I remember reading about it on the forums last month but I can't find that post again.

I haven't looked into it personally, but my understand ingto be on the safe side is that you want to get a stock rom back on the phone, and then relock the phone.

I don't believe I unlocked it. It clearly said if you want to unlock your phone be warned you won't be able to do (something or other later on.) So I figured i would wait. Anyway to verify it either way?

if you want to verify that it is locked or unlocked do this

NOTE: you will not see the SPL version during normal boot, that is the OS version, not SPL!
to enter bootloader mode to see version: with the device turned on, press and hold the volume down button along with the power button, then press the reset button with the stylus tip, then release the volume down and power buttons when bootloader tricolour screen appears.

and look got this

0.35.CMONEX or 0.37.CMONEX - if you dont see this then it is locked
