One correction I would like to make to your post.
BTAudio, which I have used forever and could not have lived without

, will actuall only send audio to bluetooth headsets, not to bluetooth stereo devices, or rather BT stereo profile devices. Whenever I use BTaudio while using my A2DP headset, it will actually disconnect the audio from the headset.
The program is awesome and does exactly what it is designed to do, which is to send all audio from the device to bluetooth headset devices.
I really like having the A2DP headsets, because it automatically routes audio to the headset, and it does work with pandora (among a slew of others), but it does not send ringtones and system sounds to the headset.
I have attached the cab below, which is the same as the BTAudio program discussed earlier
