I'm pretty sure its from overheating.
It's happened to me numerous times when I'd have the phone charging under the blanket. A soft reset gets rid of it though.

thats wat i was thinking too cause the phone ws pretty warm..... but thankfully sprint has that phone back and gave me an exchange for freewithout the isurance.
