Setting ESPT on Mogul (Sprint)

Hi All..
I'm a New user at PPCGeeks..

I have a mogul (sprint) ..
I've unlock this mogul ..
but I do not know to ESPT settings ..
I come from Indonesia my country code 510 and the network code 09
I was not able to connect to the Internet
always arise code # 1012

I have been setting up and PPP AUTH plus NAI
but I do not know what should I fill in the form

I hope the people at PPCGeeks can help me to solve my problem
