If you didnt get the form shipped to you as part of the order does this mean that they think you don't qualify for it?
I have an old Fair & Flexible 800 Family plan (2 lines) with free unlimited texting and data that I pay around $60/month after discounts. IF you read the MIR requirements from the link on the phone order page it states the following, but the actual form does not mention those requirements: PDA, BlackBerry and smartphones - Requires one of the following plans or add-ons: Everything plan Talk/Message/Data Share plan Data Premier Add-on PRO Pack Add-on |

Follow what the form says and keep a copy of the form yourself.
Pay the extra 10 or 12 cents at the post office for delivery confirmation when you mail it to them. Tuck the copy of the form you send and the delivery confirmation away fro safe keeping until you get your rebate. One of the options was having a plan that includes data. You have that and it is more than the minimum $35 a month. You'll be able to fight it if you keep a copy of the form if it even comes to that at all. Quote:

If you get denied you can always call retention and say "You guys promised me this rebate and that was the only reason why I signed up for another 2 years.....blah blah about fraud....you want to cancel even though the 30 days is up" that normally gets them to give in and offer you some more perks. I was able to do this with my 3 free Moguls when they came out plus MIR
