Follow what the form says and keep a copy of the form yourself.
Pay the extra 10 or 12 cents at the post office for delivery confirmation when you mail it to them.
Tuck the copy of the form you send and the delivery confirmation away fro safe keeping until you get your rebate.
One of the options was having a plan that includes data. You have that and it is more than the minimum $35 a month.
You'll be able to fight it if you keep a copy of the form if it even comes to that at all.

IF you read the MIR requirements from the link on the phone order page it states the following, but the actual form does not mention those requirements:
