That really depends on what RUU (ROM Upgrade Utility) you use to flash the ROM. If it's the normal one then there's no need to put in bootloader first. If it's the NOID one then yes you should put the phone in BL.
But if it's easier, you can just always put the phone in bootloader mode--that should work with any of the RUU apps. |

There's really no need to go back to stock. The extended OEM files you need are already built into the kitchen, so no need to "go back to stock to reload the extended ROM". Just clear your extended ROM and use it for storage. The CE ROM section (the part that the kitchen builds) gets completely rewritten during the load, so no point in "going back" unless your phone won't boot and nothing else has un-stuck it.
Of course, if you wan't some of the crap ware from the extended ROM, that's not built into the kitchen and you'll have to lower yourself to the stock ROM to get it again.

While I agree with gguruusa about the kitchen having a "clean" version of the carrier customizations built in, I have always found it a good idea to flash to stock before loading a new rom, and I, as well as others, have had problems which were fixed by flashing to stock then flashing to a new rom. I will admit that in no way can I prove that flashing to stock first was the only reason that the problems were fixed, but the question I have is "Why Not?" Flashing to stock and going through all of the activations brings your phone back to the way it was originally (when almost everything worked). Then, loading the new rom with the built in carrier customizations is all you need to do! I am probably one of the few who have found it necessary to flash to stock before flashing a new rom, and for most people it is unnecessary, but there is really no negative effect other than the extra time it takes.
