Yeah, dead/bad battery or dead/bad charger is when I have seen it. Well Ive only seen it personally when my battery was really dead but others have said it also does the same with the charger.

Sounds like the battery is not making good contact with the power pins on the phone.
I bought an off-brand no-name battery off ebay and it was not the same size. Was slightly smaller than OEM battery and it would not contact the power pins all the time. Thus not keeping the phone powered. Would turn off. Learned my lesson on ebay batteries....replaced it with a brand new OEM battery and then with a Seidio 1500 and have not had the problem since.
