As far as I know call forwarding does not affect your friends at all. If you forwarded your AT&T number to your Sprint number and a friend calls your AT&T number from their AT&T number, then they get the call for free (as long as they have AT&T mobile to mobile, or something like that).
Also, if someone calls your AT&T number you will get minutes deducted from both the AT&T and Sprint numbers, plus a surcharge from AT&T for forwarding the call. It basically works like the AT&T number calls the Sprint number in order to forward the call... and of course AT&T won't pass the chance of charging you for the extra service. I could be wrong, but this is my understanding of the service.

^^ This is correct. A surcharge will be charged on the AT&T phone and/or any applicable minutes will be deducted from the AT&T plan for the length of each call fowarded to Sprint. This is in effect regardless of who is calling you, as long as the phone is forwarded.
