first time i tried it remained black... seems like i tried another wallpaper or maybe from storage card or something....

Not sure who's rom that is. Sorry!
Did it already have the B4all mod cooked in?
If so, you may need to remove it, before installing MaxManila.
If so, let me know and I should be able to help you to remove it and restore your rom to function like default for wallpaper.
Maybe that would fix the issue. Maybe not. I'm not positive, but if it does have B4all cooked in, it might be worth a shot to try.

Anyone know of a 2011 slider with icons that match or are similar to the ones used for notifications on MaxManilla 3.0?

what do you mean i built a cab and installed it....selected reboot manila. it works for me.

on another note youtube app when trying to play a video errors. and says cannot play video. any ideas?

As an FYI, I finally did get MaxManila 3.0 running on Calkulin's kitchen. It was a CFC compression issue. I had to decompress 2011 manila and some options with Calkulin's kitchen, in order to get it to work. But, it is working now.
