I had the same issues but still would not work. So instead of putting the file back on the sd from my laptop I put the file on my phone then formated the sd. Then I put the file on the sd from my phone and restarted the update and it worked.

I think I remember there being some discussion about 3.42.40 being better on battery life and GPS performance, but I may be wrong. I know I have ran 3.42.40 with dcd 3.2.6 for over 2 years now on my mogul so I have attached it for anyone who may wish to try it. I believe it is set up to be flashed from a 2gb or smaller sd card in this file.
