This drove me absolutely nuts for a while. After digging into it, it turns out TF3D caches all of the shortcut menu icons as .bmg files at: Device - Application Data - HTC - ProgLauncher

Delete all of the .bmg files there and soft reset. TF3D will create new ones at startup and that should solve your old icon problem.
