USB adapter causes phone to dial?

I'm using this USB/Stero Jack adapter and everytime I plug it in, my phone dials the first person in my picture dialer

I also can't get any sound from Pandora to come out of it. I know it works as when I dial someone, I hear it coming from my car speakers but Pandora sound doesn't come out, even though Pandora is running just fine.

Is there a setting I'm missing?

I got pandora to work after hard reset but my phone still dials the last person I've called if I jiggle the usb adapter
