What rom are you using? Same thing happened to me with might/silence. I'm guessing it doesnt work with 6.1 roms?

I use v1.4. Works like a charm. i would post the cab, but i'm at work...

actually here's a link to it. Not sure if its the exact same as what I got but you can try it out.

If you can't get out of the loop where it shuts off, just go into the settings menu as soon as the phone starts and disable TF3D, then uninstall the taskbar.
