How to Check if Phone is Back to Stock + Locked?

I am getting a replacement in the mail soon, so I want to ensure that I have returned it 100% to stock and and locked it properly. What exactly should I be looking for on start-up or bootloader screen?

radio version 1.03.05f
rom 1.03
and in the bootloader mode spl-0.37 0000

should be good to go...

Whent the unit is shut off:

Push power button and down volume at the same time and hold. It will go into bootloader mode.

So, in bootloader mode I do have spl-0.37 0000 (which I believe indicates that I have relocked it properly) but my radio versions and roms are different:

ROM Version: 1.05.666.0
ROM Date: 12/09/08
Radio Version: 1.04.02V

Now, this is a Bell phone, and I am pretty certain that here in Alberta, Telus pretty much owns all the cell towers currently, so it makes sense that the radio version is the 1.04.02V (the Telus radio). What I am not so sure is why I have a 1.05.xxxx ROM instead of a 1.03XXXX ROM. Is this possible due to it being a Bell phone/ROM and that Bell got the TP later than Sprint, so it has a newer ROM?

Any insight, as always, is greatly appreciated.

Thanks! That is the dumped rom I used, but being a dumped rom and not a shipped, it does not have (or flash) the radio. That being said, none of the custom roms flash the radio either AFAIK, so as long as I didn't flash the radio seperately (which I didn't) this radio is still the stock one, right?
