Need initial Sprint to VzW instructions please

Does anyone have a copy of the original instructions for migration of the Sprint TP to the VzW network? I have reflashed my TP to the Sprint ROM for service and want to make sure I have the instructions saved.

My ESN/MEID was added to the database so I shouldn't have problems re-activating it when it comes back.

I did back up the QPST settings and AAA/HAA secret keys before I sent it out, as well as doing a full backup. I assume if I reflash to the latest VzW ROM, then load saved QPST settings and secret keys, flash to my juicy8 ROM and restore the backup file I should be golden... is this correct?
