awww i upgraded my plan on 10/11/09 and forgot to do the rebate, i'm out of luck now

If you upgraded in October you can still do the rebate as long as it is postmarked by the 14th (Saturday). That means do it today or tomorrow. Heres links to the older rebate forms that pertain to you:
