sounds crazy, but try to dip just the very tip of your jack in rubbing alcohol and then sliding it in and out a few times.

this is exactly y i don't use the 3.5mm jack something like this is bound to happen. just keep doing what others suggested .

this happens because the 3.5 jack is designed for a 4 pin microphone headset. which should be backwards compatable with standard 3 pin jacks but for whatever reason the jack clogs up and the pins bind, telling the phone that something is still plugged in, when it really isn't. you can reset it manually with a small jewelers pick to move the contact back out

I never had an issue until I flashed a specific rom, but I just did the stylus into the hole trick and that works for me every time.
It has something to do with having the usb plugged in while plugging and unplugging the headphones. Supposedly unplugging the usb before the headphones is supposed to fix it, but that didn't work for me, just sticking the stylus up there real quick did.
