Try a different 'theme'. Start, Settings, Today...
Experiment with different .tsk choices. HTC Black and MightyROM caused the error for me. I changed to something different and the icons have shown up perfectly for me ever since. The only Black .tsk I could get to work was PureBlack2 (search XDA).

My Rom=MR War 12/30
