SD ROM Flash problems with 16 gb card

It seems that when my 16 gb card was new, I could SD flash a new rom with it no problem. However, once I start to add a lot of music, my phone will never read the NBH on the root of the card. I just backed up my SD card to prepare for a format, but I'm concerned that this problem will creep up again anyhow.

Anybody have any idea why this happens? How to prevent it or fix it so that my 16 gb card can SD flash roms?
