There's a lot of debate as to whether it's hardware or software related... the bottom line is that it is a pain... This happened to me when i soft reset with the headphones plugged in. the icon was stuck, so the earpiece and mic on the phone wouldn't work since it thought they were still plugged in. I soft reset like 30 times, hard reset twice, flashed both stock ROMs, and back to Mighty, all to no avail... finally fixed it by plugging in a different set of headphones, then unplugging them... whatever on that.

a month or so ago, I had to get a replacement cuz the opposite happened. it wouldnt recognize the headphones were plugged in! that turned out to be a hardware issue.

this has happened to me about 4 times in the year i have had the phone. Soft reset, hard reset, plugging and unplugging...nothing seemed to have an effect. the last two times it has happened i found that just the right sequence of plugging the headphones back in, soft resetting with them in, then removing them and soft resetting again has done the trick...mighta just been a fluke since hardware or software, this bug seems to have a mind of its own and goes away when it wants to.

but give it a shot...

i had the same issue for a day and was sure i would have to do a hard reset or take it in to sprint. i was using the same headphones to put in the audio jack. i tried using other headphones and one of them worked. try using every set of headphones you have and one of them may do it.
