Here you go. I extracted the images out of the black and blue rom and replaced the ones in the cab scottishwarrior posted. Haven't tested it much, but it appears to work correctly. I made 2 versions in case you don't want the Earth background.
Earth Background 6 Button Black Blue Comm Manager Black Background 6 Button Black Blue Comm Manager NOTE: Make sure commanager.exe is not running in the background or this will not install.
