Reset Loop and Problems Making a Call

I've been experiencing reset loops when the SD card is inserted in the slot? Does anyone have any ideas why? I've formatted the device to start from scratch and the device started doing the reset loop again within 2 weeks of formating the device.

Also, I've found that the reset loops also cause a problem with the phone. If I hit the button to show the keypad, the numbers are whitened and I can't see or dial them. I thought that it was the dialer skin that I was using, but that was not the cause. Any ideas why?
