Does anyone know how to leave comments on a app in the android market???

Open market --> My downloads --> Choose the app you want to rate --> pick the amount of stars --> Then choose leave a comment.

one more question, how do you set a username for yourself while leaving a comment?

It is directly related to your google account. You can change that username if you would like, but that is the only way I know of.
