unroot/relock Sprint phone for TEP replacement...

I recently got a replacement authorized through TEP with Sprint. I had previously rooted my EVO with Unrevoked 3. I ran Titanium Backup and backed up all apps/system data. The transition to the new phone went well. I had all apps, settings etc. exactly as I had on the other phone within 15min. I paid for the app because it is THAT worth it. Particularly if you play with a lot of different roms.

My question is this: I'm now wondering if I properly relocked/unrooted before I sent it in. I want to avoid any potential issues if a tech feels the desire to go above and beyond and point out that my phone voided the warranty with a root.

I took the following steps before I sent the phone back:
1) Applied the unrevoked forever S-on tool (by unrevoked, see here: public:forever [RootWiki] If you look at the FAQ for Unrevoked 3 it points you there.

So I took this as the Unrevoked Forever S-ON tool worked to renable security if you used either unrevoked 3 or unrevoked foreever (s-OFF). I Applied the S-ON via recovery menu as instructed with no issue. Then;

2) Factory reset the phone. From a dialpad press "##786#" - This will take you to EPST mode with your MSL. Then press your "Menu" key on the phone and select "Reset."

Oddly enough, when I went into apps after resetting to stock rom/configuration, I saw the Super User App (lil ninja guy ). Anyway, I dont recall this being a standard app. Why is it here after resetting? Did it not get unrooted properly? Wont this app being there be a dead give away its been rooted? It's in the mail as we speak.
