no sound

I'm running a stock evo. And everytime I take my headphones
out and put them back in. Also they just randomly cut off and i cant hear my music or YouTube vids. The only thing i can hear is loud ass aim or when someone calls. I normally have to restart the phone to fix it. Is anyone else havin this problem and is there a fix
