The clock and date at the top are simi clock and ExtDateWidget respectively (both free in Market).
The weather is Beautiful Widgets with Inverted Droid theme (in the market, small fee).
Right underneath the weather is Circle Battery Widget (free in Market).
The wallpaper was shared by someone on XDA.
I made the dock and icons in Photoshop using the HeroLight font which is the same as I flashed for the system font.

And all of this is on top of LauncherPro Plus (in the market, small fee).
