SERO Diamond Rebate Form?

Did anyone get a copy of the rebate form thru the SERO phone order site? Is it different then the one offered for regular customers?

My SERO rebate forms for the Touch and Mogul ones were different, went to different departments and was proccessed differently.

Let me know if anyone has a link to it, Thanks.

it says "Rebates for the Everything Plus™ Referral Program ONLY" though

Yeap you are correct it is different going to the same dept. But it's for everything plus accounts.. Hoping to find the "sero" one..

SERO is now Everything plus from what I understand.

If I have the old $50 sero will this work? I was told by exec services that i did not qualify for the rebate because of the type of account I have.

you need the $99 plan to qualify for this... the form cant be more clear.

Form states
