I'm sending mine in. Before I ordered the phone, I asked TeleSales how much it would cost me in the end, and she included the $100 rebate. I asked the rep if I just send in the same form as my wife (who is not on the SERO plan), and she said yes. So I am sending in the normal form and crossing my fingers.
