Bad news... this appears to be a hardware problem. There have been others here who have had the same thing happen with the radio ROM disappearing. Without any other info to go on, it sounds like there is something wrong with the phone's flash memory... like perhaps it cannot be read consistently so sometimes it will read the radio ROM and other times it can't.
It's possible there is another explanation but as of yet no one has come up with one. We certainly don't know how to solve it, which sucks. If you have tried re-flashing the latest Verizon update then I don't really think there is much else you can do other than get Verizon to replace your phone. Bring it in and show them where the radio ROM says "None" and let them verify that it won't take or make calls. Tell them you've already tried flashing the latest ROM update.
Hopefully Verizon will take care of you. If not, your phone should have a one-year warranty through UTStarcom from the date of original purchase or the date of manufacture (which can be found in year/week format as the first three digits of the phone's serial number under the battery--601 would be the first week of 2006, for example).
