Here's a working module I put together for the Rom Kitchen

It's the Intent Midlet Manager. Tested with Opera Mini 3 and works great. Puts in the files, reg entries and the short cut in Program Files.

So what are the attachment rules?

I love my opera mini. I do wish the scrollbar was wider, and wish I could pageup/down with the volume button tho.

You can use left/right on the stick to page up and down.

Is this the latest MIDlet Manager v11 or the older v10?

The "About" says Runtime JTMI 1.0:v11.1.7.1023

It's the one from xda-developers from a couple months ago where Opera soft keys work.

If there's a newer version, please point me to it.

Nope, looks like you have the newest one. Definitely a great app to have whether you use Opera mini or have a gmail account.
