DFS CDMA- Sprint data settings

Hi, does anyone have DFS CDMA application settings for DATA? I installed a new ROM for my sprint phone and my data doesn't work anymore

In specific, I want to know what to fill in the UID field for PAP, PPP, HDR AN, HDR AN Long, etc.

Just curious, would updating the profile have done this?
