Micro SD card unreadable

I have an 8GB micro sd card that came stock with my HTC Evo 4G. I took it out, copied all the files, and then put it into a micro SD card adapter, then into my laptop running windows 7. I clicked on it from my computer and selected format. After a hour, I came back, and no progress had been made, so I clicked cancel. Now nothing will even recognize the card. I have tried tons of software, but I can't even get the card to pop up.. Any suggestions?

In Windows, you will need to create a new volume on the card. Do it through disk management.
