I also noticed if I do an SR that the Start button doesn't work anymore. I have to actually swipe the TAB's first before the Start Menu button will actually launch the Start Menu.

The duplicate tabs are a mistake and/or laziness on my part. Every time I add a new SYS to the Kitchen I have to exclude certain packages, sometimes I get it wrong or don't bother and then we get duplicates. Someday I'll figure out a way to automate that, but for now it's all about human error...

As far as SR leading to inactive start button, that does not happen on mine, so I'm not sure what could be causing that, other than a funky flash...

Thought so too, so I flashed back to stock Sprint ROM, HR, flashed back to this ROM and HR before doing anything more. Same result, weird.
