Are you trying to use BuildOS or BuildOS+Package_Tools?

On the last couple of kitchen builds I noticed I could not get a succesful ROM (same issue as you) using the BuildOS+Package_Tools, but if I built the same ROM with BuildOS it worked fine... Figured it was just me, but after seeing a couple of comments that were similar figured I would post my experience...

Verizon's have always been a pain to get to boot, as they don't like many changes in the OEMDrivers folder, so I may actually have to post a seperate one for Verizon users to test as I don't have a Verizon beta tester anymore

Caulk Does the fixed notification also have choices for snoozing reminders instead of the worthless default 5 minute? I saw that Dutty's ROM has this for his fixed notifications. Thanks!

I'm on Sprint, so I don't think it's just the verizon carrier files...unless there's a problem with the sprint files too.
