Could be wrong but it's likely MaxSilverRoot. The 1st pic in the quote has the MaxManila Alert icons, Call History is an available option in the MaxManila Settings, and jdm has been on that ROM afaik.
I'm on that ROM right now and have my Home screen looking like that.
It is hard to keep it straight with all the options root throws at us

there are many clock cabs you can use to replace the clock.

@tb5-not sure what exactly you find ugly but if it's the graininess of the clock, it is more pronounced in the screenshots than on the device, just a fyi.
