ive had that happen before with my bt headset but this was after a call when i hung it up using the end call button rather than the headset button and that routed the sound back to the device even tho the headset was connected. but if this is happening with a wired solution the headphone jack must be going (as they commonly do ) i use my micro usb headset from my tp so i dont have to worry about my jack going anytime soon. try resetting and flashing a stock rom if the problem is still there then its a hardware problem.
