I think I replied to ALL emails (or at least tried hard to do that - that request page just sends an email to both of us for now, but we are upgrading the system soon).
anyway, the answer to that question is,
IF you donated BEFORE the hardspl/simunlocker release,
then send me the request with this info:
a) transaction ID for donation *and* your paypal address you used for it
b) IMEI
c) email address to deliver the sim unlocker to
if you donated AFTER it then unless a significant amount, better if you just use our site to purchase it, at
http://rhodiumw.htc-unlocks.com because that is an automated delivery system, so it's easier and faster for both of you and us
