Volume changes when in headphone mode

This is the same issue I had with my mogul (which I never solved)...

Here is the way the phone is set up:
Touch Pro with the USB/headphone adaptor, then a 2.5-3.5mm adaptor, then a tape adaptor into my car's tape deck.

If I start from a soft reset the tape adaptor plays at a good level, about in line with a radio station, when the system vol on the phone is at about 75-80% (higher then that it gets distorted).

When a phone call comes in (and then I plug it back in to listen to music) the system volume becomes much louder (no idea why) and I have to turn the system volume down to about 25% to get no distortion.

Now the really weird part - it stops playing through the tape deck after a random amount of time. It won't start again until I do a soft reset, and then I need to turn the volume back up to 80% on the system volume on the phone.

It's very annoying - If I had to guess I would say that it is getting too high of a signal level and that's why it cuts out.

Anyone else?
