I just had this happen to me when flashing a new ROM. After two weeks and much pocess of elimination I figured it out. The ROM was fine, I finally found that it was caused by me using Titanium Backup. When I restored my SMS and MMS it caused the problem. The data is labeled as "[SMS/MMS/APN] Dialer Storage 2.2". To get around this, I used Titanium to restore everything but the SMS/MMS, and I insted used Sprite backup to restore my SMS/MMS. This solved the problem completely for me.

FWIW, I just flashed a new rom and was unable to send or receive mms. Tried multiple times to update PRL and profile to no avail.
Last ditch before a hard wipe, I did ##3282#, edit (have to have msl), data profile, then menu>restore. Rebooted the phone, allowed it to update the profile and prl, two waiting pics came right in.
