Process of switchin to verizon. need help.

can anyone point me in the direction of steps i need to take to get my mogul operating on my verizon service.

i just recently had my sprint service cut off, and i now am with verizon, however, my mogul is set up for sprint n has a sprint (nue)ROM on it.

i have all my info for both phones (the mogul, and the temp phone i have on verizon), but im just confused on where to start. any help appreciated.
