Make sure you name the file TITAIMG.nbh

This is what I was forgeting to do a while back.

1. Using a PC, format a microSD card to FAT32.
2. Using winrar or an unzipper, extract the RUU_signed.nbh file from the ROM upgrade .exe.
3. Copy the *.nbh file to the root directory on the formatted microSD card
4. Rename the file on the microSD card TITAIMG.nbh.
5. Insert the microSD card into the phone and reset into bootloader (power, camera, reset).
6. Follow on-screen instructions on the phone.

This apparently requires the phone to already have the appropriate unlocked bootloader installed (Olipro 2.40 hard-SPL in most cases). Some users have been unable to use SDHC (4gb and up) cards.

Also, I have seen mention somewhere that the rom you want to flash has to be the ONLY thing on the card.

As long as the ROM to be flashed is properly named and in the root directory, the card may contain other files.

Use a different card.

I had the same problem with a 1 GB micro SD Kingston card. I did the same exact thing with a 2 GB A-DATA card and it worked. Tried the Kingston again, and it didn't work. So there must be something with certain cards.

Both cards were NOT SDHC, so that wasn't the issue.
